The Sino-Nasal Outcome Test (SNOT) is intended to help define your symptoms and provide valuable information and insights for Dr. Gottlieb. This questionnaire is used to help us establish how your chronic rhinosinusitis (CRS) has affected your health status. The survey will also measure the physical impairments, functional limitations, disabilities, and societal limitations caused by your sinusitis.
Dr. Morris B. C. Gottlieb has been in practice in the North Dallas area for over eight years. After receiving his B.S. in Molecular Biophysics and Biochemistry at Yale University, he completed his M.D. at The Johns Hopkins University School of Medicine. He completed his surgical and subspecialty training at Duke University Medical School. Dr. Gottlieb is board certified and is a Fellow of the American College of Surgeons, a distinction that very few of the ENT doctors in the Dallas area have achieved. Dr. Gottlieb has been the proud recipient of the Patient’s Choice Award in 2010, 2011 and 2012, one of only a handful of local physicians to earn this great distinction. This award is based solely upon patient opinion.
